Email Integration with Microsoft Outlook Not Adding the CRM button to Outlook

I have a user that has installed the email integration package; however, the Add to CRM button does not appear on his Outlook toolbar.  I uninstalled and reinstalled on my lapton 3 times without issue; the button appeared each time.  Has anyone experienced this and do you have a fix?
