Change Order auto promote fails but there are no error or pending actions

Recently we are having new issues come and this is one of them.

ECO auto promote failed, but there is no erroe or pending actions required from the user side.

All the approvals has been done and there is no entry or exit criteria missed.

Still there is auto promote faliure.

There was also a case where the ECO was in review and all the pending approvals done still the ECO wont move to release.

Strange in this case was there was no auto promote faliure but when the user moved this ECO manually it never throwed any errors also.
