21CFR11 P9500005

I am setting up the 21CFR11 according the the E1 Auditing and Electronic Signatures guide for path code DV812.  The changes are being logged in the Axxxx table.  I used UTB to verify that the changes are recorded correctly.  The issue I am having is with P9500005 [Search Audit Info].  I receive an error "Error Fetching Audit Record" whenever I select a field (Unique Identifier, or Program Id, etc...) and click OK.  The F9500005 is mapped to Local - DV812.  Is this the correct OCM mapping?  The table F9500005 is not being populated.  What could be causing this error?  My
