Error after upgrade from R12.1.1 to R12.1.3

Dears

I did the upgrade from R12.1.1 with database vrsion 11.1.0.7 to R12.1.3 with database vrsion 11.1.0.7 successfully with status autopatch complete and no error and i started all services of application without any problem and i did adautconfig for databse and application and recompile apps schema and relink all from adadmin

login in to application it is ok no problem

show responsibilties and it functions ok no problem

open window i get this error when open some windows ::::

no data found

when_new_fom_instance

form trregier unhandled exception

Kindly any suggestion to resolve this issue
