Unable to create lines from agreement to sales order ( Error:FRM-40700: No such trigger: DISABLE_HDR

Hi All,
I am trying to create a sales order from sales agreement but the lines in agreement alone are not copied to sales order.

All the header information is copied and the below error is displaced in the status bar.

 FRM-40700: No such trigger: DISABLE_HDR_LIN_ITEMS

EBS version : 12 .1.3
Form version : OEXOETEL 11.5.100
                           OEXOEBSO 12.0.49.12010000.4

Any pointers on this error is highly appreciated.
