How to solve "The percentage of requests that resulted in errors is 17.06%" situation

 Here's the situation : Brand new EM12c installation running on a single node (DB and Middle Tier on same server) on Enterprise Linux 64bit. Installed using the Oracle EM12c installer taking the simple install. Everything is OK -  I've added the Database using auto discovery and everything is working fine... .apart from 1 incident which is being raised by the metric  " ohs_server:error.currentPercentage"  against EM's own web server ( /EMGC_GCDomain/instance1/ohs1(Oracle HTTP Server)). The summary is " The percentage of requests that resulted in errors is 17.06%" - and the actual error rate is running at about 26%.

Digging into the log files shows this error:
