Change of supervisor on PMP

Hello,We are using R12.04  and we  are at the final evaluation of individual performance, unfortunately there a lot of changes  in the reporting hierarchies. for example, some employees supervisors' have changed and some supervisors have left thees job.
Please assist how we can take appraisal to respective supervisor and for those supervisors who left how can we take their surbodinates he right supervisor!
