Autocreate table

Does anyone know where i can find a list of possible issues that would cause reqs to keep getting hung in the autocreate table?   All the ones i am having issues with have a BPA and are being generated from ASCP workbench.I have verified most of them with ASL setup and valid BPA, but i have a few that look correct but still get hung.
