Problems with re-enrolling into a plan elected previously?

Has anyone had a problem re-enrolling an EE into a benefit plan they had previously? Let's say a person elected SGL medical coverage, then went to DBL, then wanted to return to SGL. These choices happen over a matter of years. OCCASIONALLY, but not always, we can't get the person back into the original (in this example SGL) plan option. This has happened with medical and with dental. It has happened with various options: SGL, DBL, FAM. We haven't been able to find rhyme or reason why this happens once in a while. Our only recourse is to delete (SQL)
