To show sites of a customer in SR form.

Hi,
I have made a new customer with 2 sites.
In 'Create SR' form, I am selecting that customer. Can I select site there for this customer, as in our case customer name will remain the same and sites can be more than one for a single customer.

In 'Address' field, address is coming which I have defined during making customer.
Can I change site for a customer in SR form, so that user can differentiate by addresses of all sites.
