WA State General Liability

With the WA State Industrial Insurance calculating on hours worked, how do you set up the General Liability which is based on percentage of earnings?  We cannot use the same WC Code with -0- for WC and the the percentage for the General Liability - JDE says do not use duplicates....  Do I need to set up a DBA and assign to all WA emps?
