Create Task Workflow - task due date = 2 weeks from date selected on custom Opportunity date field

I am trying to create a workflow for the opportunity page. I created a custom date field called "Assigned to Dealer Date".  I would like the workflow to create a task with a due date of 2 weeks from the selected "Assigned to Dealer Date". 

Sorry I am not good with syntax/expression, what should I use for the Workflow Rule Condition?

For the Actions I would choose a Wait Action, what should that Expression be set to?

And for the Create Task Action, Do I also need an Expression for Due Date?
