Unreliable SNMP behavior

We're running WLS 10.3.4 on OEL 5.4/x64.
Attempting to configure JMS queue counter monitor. 

Simulating a breached threshold using a 3rd party tool to post messages to queue.  Works great.

Set up snmp agent to throw traps to a local listener (java weblogic.diagnostics.snmp.cmdline.Manager SnmpTrapMonitor -p 162)
*Obviously this is for testing and config, with the intention of pointing to our trap manager if we ever figure this out.

I believe that both my agent config AND counter monitor configs are kosher, because I've gotten this to work.  I set threshold for 2, send 3 messages with posting tool, listening trap monitor throws:
