ORA-01017: invalid username/password

Hello, please any help about EBS R11?

After login with sysadmin user, application request change password. When I input new password I get error message "ORA-01017: invalid username/password"

In table apps.fnd_user "SYSADMIN" user is active

When I run AOL/J Setup Tests, I can connect and utility show current session settings, but when press show responsibilities message "Null returned for responsibility list"

I can't do any login

If another user try to connect, get message "Login failed. Please verify your login information or contact the system administrator"

For additional information we have policy security active in APP (Profile: Signon Password)
