Risk involved during node addition or deletion

 I have couples of questions related to rac node addition /deletion and os patching  in windows 2003 server(64 bit machine).Can someone help me to get answers

1. What are risk involved during adding a new node into existing cluster? can we performed this activity without down time of the production system.
2. What are risk involved during deleting a existing node from cluster? can we performed this activity without down time of the production system.
3. what are the pre requistics(interms of hardware,os,software) to delete one node from  one RAC database and added into another RAC database.
Your quick response will be highly appreciated.
