US PAY: FEDW211.mf rejected when uploaded to SSA through AccuWage

Hello,

Our payroll administrator tried to upload the FEDW211.mf to SSA website via its AccuWage system.  The file got rejected. One of the error message is about not having a valid contact phone number. We checked our GRE setups and from what we can see, we do have valid contact person name, email and phone number in Oracle. 

What would have caused this issue?
