How to handle upgrade of source instance to R12 in a decentralized ASCP environment.

We have a decentralized ASCP. Currently, the destination instance is VCP Rel 12.1.3 and the source instance is in R11i. But we are now going to upgrade the source instance to R12. Is there any document that has a check list of what needs to be done in VCP and in the upgraded source instance inorder to have a successful migration?
