E-Business Suite Upgrade from 11.5.10.2 to 12.1.3 and Oracle EBS Single Instance database conversion

We want to upgrade Oracle E-Business Suite Instance from 11.5.10.2 to 12.1.3. We also want to convert Oracle EBS Single Instance database to Oracle RAC. What should be the best practice PATH to achive this goal? Thanks
