Requisition Creates Multiple POs - SCM 9.1/Tools 8.50

We have a sporadic issue where a Requisition (created in ePRO), once through the Auto Source process, continues to have lines where the SOURCE_STATUS = 'A' (Available) and QTY_OPEN <> 0. The PO is created fine the first time, but since one line on the Req continues to show as available, multiple POs are created from it. We had this issue sporadically before the upgrade to 9.1, and see the same issue now. Wondering if other users have experienced this. We have a case open with Oracle, and numerous searches of the knowledge base have returned no results.
