System cannot determine geographical information for this location and cannot derive a tax jurisdict

I am getting the message "System cannot determine geographical information for this location and cannot derive a tax jurisdiction" when entering an transaction.  when I ran the script provided in doc. 438718.1, the map_status = E. 

1) I ran Geography Name Referencing Program for HZ_LOCATIONS for all and the result 

Total number of Processed Locations:193619
Number of Successfully Processed Locations:193619

2) Navigate to Trading Community Manager -> Administration -> Geography Hierarchy

Click on Manage Validations, and find the geography mapping and validations.
.
Only State is enabled for Tax and Geography validation because we only intend to calculate tax at state level.

3)  All customer addresses have valid state.
