To display off 'Open Folder' icon on ceate SR form.

Hi,
Can anyone tell me internal name of 'Open Folder' icon (left top corner in create SR form CSXSRISR), so that I can display that off using form personalization.
I have tried using 'INCIDENT_TRACKING.FOLDER_SWITCHER'--'DISPLAYED'---'FALSE' and 'FOLDER_OPEN' on 'WHEN-NEW-FORM- INSTANCE' trigger, but it is not working.
Can anyone help me on disabling it on create SR form (using form personalization or any other means).
