Drop disk from one diskgroup and create a new diskgroup

Successfully dropped a disk from a disk group that was used for FAL.  However, upon adding it back as a new disk group it was only visible on one node.  re-dropped the disk successfully.  Now the header_status shows FORMER, not candidate.  I was under the impression that when dropping a disk that it would reset the header.   An error wasn't received when the disk was re-added.  I can find no indication as to why the disk group wasn't visible to the other two nodes.  Also strangely, when a file was created on the disk group, (the DBA didn't check to
