Q&A in 2 columns layout

Hi All,

I had an opportunity to lead On Demand Marketing workshop for end-users (benchmarking group).
They raised interesting question, which concerns web form and Q&A in 2 columns layout.

I attached an example of a survey question, which provides a kind of comparison. I’d like to avoid using two separate survey questions. 

Is it possible to meet such requirement, use reasonable workaround or address such product enhancements for ODM?

I’ll appreciate good tips or idea to address such req.
