Trouble Associating Custom Object 05 with Leads

I am having trouble configuring Custom Object 05 so that I can associate it with existing Leads.

I have done the following:

(a) Exposed the Leads picklist on my Custom Object 05 layout, however it is not editable.

(b) Exposed Leads as a Related Item on my Custom Object 05 layout, and there I have a NEW button to creat a new Lead, but not the ADD button to associate the CO 05 with an existing Lead.

I had no trouble setting up CO 04 to do this, but I can't figure out what I'm missing on CO 05.

I've checked my Default and Owner Access Profiles for the CO 04 and CO 05 record types, they seem to be the same.  The Related Information configuration for Leads for each record type also seems to be the same.
