Exadata cell nodes issue on version 11.2.2.4.2

We are applying the 11.2.2.4.2 on cell nodes.It got failed with below error

Mar 26 20:42:46 [INFO] Check healthy states of Bootable or/and MD device(s)
Mar 26 20:42:58 [ERROR] Bootable or/and MD device(s) do not seem correct or consistent.
