Unable to create a customer contact

Migrated from 11.5.10.2 to 12.1.3 and when creating a contact to an existing customer getting the below error.

HzPuiCustAccountRoleId

Found note # 470342.1 We have already applied that patch and also ran the DQM programs still getting the error.

Any suggestions.
