PM not generating work orders

We have problem with PM work order generation. Have two bi-weekly PM's, one for 16S-1938 and one for 16S-1939.  Both use Activity "2WK PM FLARE POTS".  There were no Work Orders generated for April or May.

What could be the reason for the failure of work order generation.
