Bi publisher mapping tool

Hi

Im frankly new to the Bi Publisher administration and development and im having trouble using the bi publisher mapping tool. i followed all the steps up to generating a sample data file which was successfully generated but the problem lies when i click on the "map form fields button" another windows appears as it should but appears blank with the messages "Click on each highlighted form field and select the data field that it corresponds to. When finished, use the Submit button in the PDF form".

please help?
