Approval Framework 9.1 - Request Information Added event is not creating the email notification or w

I set up Requisition Approval Workflow using the delivered Transaction Configuration. The Approval Process that I configured is at the Requisition Header Level and the Notification Options are: enable email and worklist and the notification strategy is Online Processing. All of the worklow events are generating the email notification and worklist entries except for the "Request Information Added" event. Has anyone come accross this problem?  We are on Tools 8.51.12 and Application bundle 12.  Thanks!
