how to add database RAC instance to GRID control

I have 2 nodes that i installed RAC and all finished successfully but when i setup agent on both nodes and try to configure it with exisitng GRID EM i cannot see database objects and i cannot manage database

Please advise how to solve this issue or give me the details steps to setup RAC and configure it with GRID EM
