Smart View Tool Bar Disappears

We are currently using Smart View version 11.1.2.  The issue that is being encountered is that the Smart View Tool Bar disappears for different users randomly.  It is not always the same users that it impacts and is not always at the same time.

In order to fix the issue when it occurs, the user peforms the steps included below, and then the tool bar reappears. 

In Excel 2003 do the following:
Menu > Help > About > Disabled ItemsAnd make sure that no Smartview Comp is disabled. If there are any Smartview disabled items, please enable them (right Click enable...)The question is, is there any way to prevent this issue from occuring in the future.  It occurs for users with Excel 2003 and Excel 2010.
