Implement Approvals Functionality for Order Type

Hi all,
I am having issues implementing the approvals functionality for a transaction type. I want to create a RMA - No Shipment Order type and I want 2 levels of appovals. However when I assign the approvals they dont fire some reason.

Why may this be happening?
