what are the tables assocfiated with the contract approval cycle?

Problem Description: WIthin the Service Contract module, we have an approval cycle whereby contract goes through statuses of QUotes, Submiutted, Rejected or Approved then lastly - Active once approved.
We need to provide a report (using sql) that displays contracts and all the phases of approval along with datestamp and approver and notes that can be found on
Can you please point us to where we can find this information (located on workflow notification (Worklist)) with the correct tables so it can be queried to provide a SQL report.
