Missing Internal (ICM) and Standard managers following 11.5.10.2 to 12.1.3 upgrade

Anyone seen this one before? We upgraded from 11.5.10.2 to 12.1.3 (via 12.1.1) and we do not have an ICM or Standard Managers following the upgrade.

I've been working an SR for weeks now with little luck.

I ran the following FNDLOAD to recreate the queues and it's erroring off on ICM seeded data:

FNDLOAD apps/apps 0 Y UPLOAD \oracle\PROD\apps\apps_st\appl\fnd\12.0.0\patch\115\import\afcpque.lct \oracle\PROD\apps\apps_st\appl\fnd\12.0.0\patch\115\import\US\afcpque.ldt
