Missing Batch Headers

Is anyone having a lot of issue with Missing Batch headers? I have missing headers for AR, AP, and GL.  This happens two or three times a week.  More noticeable with AP batches. I have asked the user to put a number in the amount and document count on the header when he or she creates a batch but still the batch header will go missing.  I am aware of how to create the header and also have the integrity report to rebuild the headers but is there anyway to stop it from happening?  We are currently on 8.12 tool release 8.97.
