How to default additional information while creating work order based on the asset in self service p

Hi,

     We have some assets like equipments. There is a requirement that whenever a work order is created for these equipments, certain additional information like Make, Manufacturer, Capacity details should be defaulted in the Work Order screen from the details defined in Asset master as Asset Attributes. These info is already captured as asset attributes. How to default  the same.

We opened a DFF in Wok Orders. These details are getting defaulted based on a form personalization which we did in the Work Order Creation Form.

But this is not getting applied when the user tries to create a work order using  Self Service Page for create work orders.
