Workflow error while approving requisition

Dear all,

I am having the following issue we are using 12.0.4 with AME at the second level aproval , the system displayed the following error and requistion status was set to incomplete

I checked the supervisor heirarchy and it was fine and I used test work bench in AME , no errors were shown in the tes work bench . please help me
