Deployment (Falsely) Failing due to Duplicate Members

Hello

Recently I had LCM'ed out of dev an accounts dimension and into the production environment for our Transportation application.  The initial deploy works, but any subsequent deploy errors out due to error message 1060051 (member name already used). I searched within EPMA for that account name, but only one record exists. 

Any insight as to why planning is recognizing duplicate members when I can't seem to find them?
