Copy Service Request function in R12.1.3 Service desk agent dashboard returns error page during look

I'm experiencing an HTML page error when using the copy S/R function in R12.1.3 Service Desk Agent Dashboard.  The error occurs when you try to change the contact name in the Contact information region after selecting the Action "Copy Service Request" and click the go button.  The HTML page error occurs if you "TAB" out of the contact name field.

Another anomoly is that you have to enter the contact name field in FirstName LastName format in order to see a search result if you click on the magnifying glass.  This seems to be inconsistent with the way the field is displayed in LastName, FirstName format.  Even when you select a contact name from the search result screen, it is not populated back on the S/R screen.
