Patch 13957942 Payables 12.1.2 was Applied and the Form CSCCCCRC.fmx (Oracle Teleservice) can not be

When applying 13957942, we observed the following:
The following Oracle Forms objects did not generate successfully:
              csc     forms/US        CSCCCCRC.fmx
The fmb was not part of the patch, so there must be some dependancy that changed.  Will upload the patch logfile along with the adadmin log and the worker log showing the reason for Form not generating.
Please help the customer to have that form generated.
