Unable to Save New Record in Master Items INVIDITM

We are in the process of upgrading EBS from 11.5.10 to 12.1.3 and I am encountering a problem creating a new item in the Master Items form. Details from my support request as follows:

Problem Description: Attempt to create and to save new record, no message indicating record saved and no error message generated. Exit form and requery record not found.

### Steps to Reproduce ###
1) Inventory>Items>Master Items
2) Enter Item number, Description
3) Tools>Copy From Template>Done
4) Fill in Required DFF
5) Save
