insert request_id and program name into customize table through personalization

Dear All,

              I want to pass request_id and program name when user press button of view output in request form and I want to did through personalization. Suppose I create one customize table and want to insert value of request_id and application program name every time when

user click the view output button. Please give me and help me elaborately.
