How to investigate and solve "Change password failed"?

Dear Sir,

        Whenever users forget thier password and the admin do reset password at the unix(AIX). Then user login to Siebel with the initial password. They will got the change password screen pop-up. But the "Change password failed" has occurred very often on both test and production Siebel. Only sometime user can successful changing password after try many re-set and/or try on many client PCs.

        Could anyone help advice me what the step to investigate root cause of this problem and how to solve?
