Patch 6678700: 12.1.1 Upgrade Fails on HR_API_USER_HOOKS_UTILITY

upgrade from 11i to R12.
while running patch 6678700 he has found the following error in  4 of the workers:

ERROR

ORA-06501: PL/SQL: program error
ORA-06512: at "APPS.HR_API_USER_HOOKS_UTILITY", line 891
ORA-06512: at line 1
