Supplier Profile Audit-How to capture evaluators comments

Hi

We are on R12.13 and planning to have SLM.We have re-evaluation process for supplier with a frequency of 1 or 2 yrs.

We intended to use the Supplier profile Audit Negotiation style RFI for this Re-evaluation process.

Once the supplier provides the response,the RFI creator then uses Manage colloboration Team feature to add couple of Internal evaluators and wanted them to provide detailed evaluation comments.

For internal evaluators we create a task and provide the target date.

Now the questions

1.How do we make the evaluators provide Evaluation comments in a detailed format-I saw Abstract & forms feature but not sure how to create this with specific evaluation comments format.
