Standard Purchase Order Form, Error ORA-01403 Received

When trying to access the Standard Purchase Order Form for the first time in a test environment, I am receiving the following error:

ORA-01403: no data found

--------------------------------------------------------------
FRM-40735: PRE-FORM trigger raised unhandled exception ORA-06508

Does anyone have any idea on how to remedy this error?
