Patch 6678700: Patch 6678700 fails with all workers

Hi,

Customer is in the process of applications upgrade from 11.5.10.2 to 12.1.1 and while applying maintainance patch 6678700 all workers are failing. Uploaded the log files for your reference.
Please suggest the next action plan.
