R12 - Initiator Organization LOV is blank

Hello ~

We have a user that needs to create document in Desktop Integrator (ADI) but the initiator organization LOV is blank for her.  She recently transfered to this department and the other users who were in the department when we launched do not have this issue, so I assume we are missing a access flag some where, but I do not where.

Advise?
