If deployment fails at step 2 or 3 and you check the log files you may see...     

WARNING: Node <db02> is still not         pingable even after adding its ip <99.8.77.66> to /etc/hosts          #Step 2#
         WARNING: The node <db02> is seeing 100% lost packets on         ping test. #Step 2#
     

      When pinging the same IP from another terminal the host is       reachable and then subsequently resuming the install with       GridInst.pl everything works fine.
