xml publisher report template failing

I have updated a report template use Office 2010 with BI Publisher version 11.1.1.5.0, when updated I can preview the data successfully; however when I register this template in the app (we are running 11i) the report fails with a invalid namespace error. It says namespace "ref" used but not declared. The bizarre thing is that when I update the template in word and DO NOT upload the data but simply upload the new template I do not get the same issue, the report completes successfully. This workaround is ok for making small changes but will not work for creating a report
